I'm not quite sure what you're asking here.  If you want to add new shapes
to Dia, there are two ways (in v. 0.90):  

The first and easiest, but also the most limited, is to design the shape in
Dia, then export it as a Dia shape file, then use the Sheets & Objects
dialog to add it to a sheet.  The pixmap (a png, actually) will have the
same filename as you exported the shape as, but with .png extension.  You
will need to edit the png, and probably will also need to edit the
connection points of your shape (in the .shape file, which is just XML).

The other and more difficult gives you much more power, and that's what
relationship currently uses.  In this, you make a program (like
objects/UML/relationship.c), add it to the appropriate library (by adding
the source file name in Makefile.am), and recompile.  You also need to add
a pixmap for it (again, .png is good), and then you need to add the object
and icon to the sheet (sheets/UML.sheet.in).

You should try using the export-as-shape method first, it's a lot simpler.
